Membership Requirements for small businesses:
1. Not less than 2 or more than 48 employees
2. *75% participation by eligible employees
3. Minimum 50% employee only contribution by the employer
4. Provide the most recent wage and tax quarterly report at time of submission and at least on an annual basis to the carrier. If the only employees are owners and do not appear on the wage and tax report a copy of the articles of incorporation, a recent company bank statement, and other documentation may be required by the carrier to prove eligibility
* An eligible employee is full time, working at least 30 hours per week, and not covered by GROUP health insurance with another company.
